To order the card, please use the form below. You will have to mail to us a check with your payment. You will receive the card with special plastic cover. You will have to cut your picture to the proper size, slide it beneath the cover and seal it together.
The card costs $22 plus $3 issuing and mailing fee. You MUST be below 26 years old to be eligible for a card.
